
Jacklyn D. Rose, APRN-FNP | Nurse Practitioner | Sapulpa, OK
Originally from Enid Oklahoma, Jacklyn D. Rose, APRN-FNP, moved to Tulsa in 2004 to attend ORU where she received her bachelor’s in nursing. After being a critical care nurse for 10 years, she immediately saw the importance of preventative care. Therefore, she then went back to school to receive her master’s in nursing from the University of Oklahoma at Tulsa. Overall, Jacklyn hopes to empower people to live a healthier life. Preventative care is her biggest passion. Her hope is that patients realize they have more power over their health by the decisions they make daily. She is hoping to bring her expertise of heat stroke prevention, stroke care, and heart disease to the city of Sapulpa.
When Jacklyn is not helping others, she enjoys outdoor activities with her family, especially going to the park with her kids, playing volleyball, and softball. She also enjoys many arts and crafts like; baking, sewing and making wreaths. She is a big sports fan. When she is not cheering for OU, her entire family is rooting for the Cincinnati Reds.

The Axis HealthCare care clinic in Sapulpa is located off of E Taft Ave near S Park St and S Main St. Our urgent care clinic is across the street from Kelly Lane Park. We are located in the shopping center with the Dollar General. You can find us in between Sun Touched tanning and Petsense.
