Patient Rights and Responsibilities

Axis Healthcare is committed to providing respectful, high-quality, patient-centered care. As a patient receiving services
at Axis Healthcare, you have the following rights and responsibilities.

Your Rights as a Patient

  • To be informed of your rights and responsibilities.
  • To receive reasonable access to care, treatment, and services.
  • To know the identity and role of healthcare professionals involved in your care.
  • To privacy, confidentiality, personal security, and personal dignity.
  • To receive care in a safe environment free from abuse, neglect, or harassment.
  • To receive considerate, respectful, and culturally sensitive care.
  • To receive impartial care regardless of age, race, gender, national origin, religion, disability, or other status protected
    by law.
  • To be informed about your medical condition, treatment options, risks, benefits, and alternatives.
  • To participate in decisions regarding your plan of care and provide informed consent.
  • To refuse treatment, except as required by law, and to appoint a surrogate decision-maker if unable to make
    decisions.
  • To effective pain assessment and management.
  • To be informed of treatment outcomes, including unexpected outcomes.
  • To formulate advance directives and have them respected in accordance with the law.
  • To receive care whether or not an advance directive is in place.
  • To access spiritual or pastoral services when available.
  • To participate in or refuse participation in research or clinical training.
  • To access protective services when needed.
  • To confidentiality of medical records and timely access to those records.
  • To receive an explanation of your bill regardless of the source of payment.
  • To request transfer to another facility if services are not available at Axis Healthcare.
  • To express concerns, complaints, or grievances without fear of retaliation.


Your Responsibilities as a Patient

  • To provide accurate and complete information about your health and medical history.
  • To follow the agreed-upon treatment plan and ask questions when instructions are unclear.
  • To respect the rights, privacy, and safety of other patients, visitors, and staff.
  • To keep appointments or notify the clinic when unable to attend.
  • To follow Axis Healthcare policies and procedures.
  • To discuss financial responsibilities and provide timely payment for services.

If you have questions about your rights or wish to file a complaint, please contact Axis Healthcare. You may also file a
grievance with the Oklahoma State Department of Health or other appropriate regulatory agencies.
