A native of Tulsa, Dr. Hubbard attended Tulsa University with a Major in Biology and Minor in Chemistry. After graduating TU, he attended OSU Medical Center for Health Services, where he received his Doctoral Degree. He completed his Internship at Tulsa Regional Medical Center and his residency in Family Medicine in Durant, Oklahoma.

Starting out with Axis in 2014, Dr. Hubbard saw patients in our Pryor and Inola offices, but his want to be closer to his family helped him make the decision to transfer to Bixby. His passion for his family, healthcare and helping people really show in his work. Whether it be treating every patient like family or taking the time to address every concern, he prides himself on making lasting relationships with his patients.

Dr. David N. Chorley, D.O., originally from Wednesburg, United Kingdom, specializes in Family Medicine, Geriatric Care and Abnormal Disease Control.  As a lover of rare and exotic diseases, Dr. Chorley starting treating and educating on a disorder named Ehlers-Danlos Syndrome or EDS. He received his first case in 2005 while working at St. Johns Sapulpa and from that point forward, he knew it was his mission to help and bring awareness to this special group of people. From 2005 until present, Dr. Chorley has diagnosed and/or treated over 300 patients, who to the untrained eye, seem relatively healthy.

After living in Dallas, Dr. Lackey decided he wanted to raise his family in the country side and did not want to work in the hustle and bustle of the city. So he and his wife moved to Oklahoma. When asked why he chose this area, Dr. Lackey said, “I wanted to live and provide quality Family Medicine in a rural setting.” To give the underserved the care and education they need is something he feels very strongly about.

Dr. Lackey not only acts as a Family Medicine Primary Care Doctor, but he also is trained in Family Planning. He studied NaPro Technology. He can identify problems couples are having while trying to grow their families and can either medical treat or educate them on natural and effective ways to conceive for a fraction of the cost!

Dr. Tony Hung Quan Ho, M.D., a native of Saigon, Vietnam, attended 7 years of medical school at the University of Lausanne in Lausanne, Switzerland and completed residency at the University of Oklahoma College of Medicine.  

Dr. Ho has treated patients in the Sapulpa area since 1985 and has been with Axis Healthcare since August of 2006.  Dr. Ho enjoys playing the piano, gardening, playing the guitar, riding motorcycles and dirt bikes, swimming, wood working, and housekeeping!

Dr. Ho is a firm believer in staying physically fit and taking good care of one’s body.  With 30+ years of experience in Family Practice, his passion to take care of those in need continues to grow to this day. That is why he is still accepting New Patients at our Sapulpa location.

Julie Broaddrick, APRN-FNP

Julie is a board-certified Family Nurse Practitioner seeing patients at the COVID-19 drive through. She is originally from Fresno, California and moved to Tulsa in 2006 where she completed her bachelor’s in nursing at Langston University in 2015. She obtained her master’s in science at South University in Savannah, Georgia. In the last 6 years, Julie has specialized in cardiac intensive care, labor and delivery/women’s health. She pursued her career as nurse practitioner in 2018 because of her passion invested in the care and treatment of those she can serve. 

Julie is committed to getting to know her patients and building relationships to better understand her patients. She believes in individualizing care to each patient needs, and enjoys educating patients and providing them the tools to successfully manage their health. She takes on a holistic approach to medicine through preventative health, allowing better response to improve patient outcomes for optimal well-being. Julie hopes to impact the lives of others by supporting and working with them through sickness and health.

In her spare time, she enjoys cooking and spending quality time with her family and two boys. 

Jennifer Adoh, APRN-FNP

Originally from Sapulpa, Oklahoma, Jennifer moved to Tulsa in 2005 to attend Langston university where she received her bachelor’s in nursing.  After being a cardiac nurse for 13 years, she went back to school to receive her master’s in nursing from South University in Savannah, Georgia. 

Jennifer considers herself a people person and enjoys being able impact patients and their lives in a positive way working with them during times of both health and illness. Jennifer has a passion for providing her patients with the tools and knowledge necessary to successfully manage their illnesses and chronic diseases in order to live their best lives. Jennifer’s approach to medicine is holistic and her goal is to help each patient achieve the highest level of function and self possible. She also believes treatment plans should be individualized and based on the needs of each person in order to achieve optimal benefits. 

In her spare time you can find Jennifer curled up with a good book or spending quality time with her family and 3 children.

Originally from Enid Oklahoma, Jacklyn D. Rose, APRN-FNP, moved to Tulsa in 2004 to attend ORU where she received her bachelor’s in nursing.  After being a critical care nurse for 10 years, she immediately saw the importance of preventative care.  Therefore, she then went back to school to receive her master’s in nursing from the University of Oklahoma at Tulsa.  Overall, Jacklyn hopes to empower people to live a healthier life.   Preventative care is her biggest passion.  Her hope is that patients realize they have more power over their health by the decisions they make daily.  She is hoping to bring her expertise of heat stroke prevention, stroke care, and heart disease to the city of Sapulpa.

When Jacklyn is not helping others, she enjoys outdoor activities with her family, especially going to the park with her kids, playing volleyball, and softball.  She also enjoys many arts and crafts like; baking, sewing and making wreaths.  She is a big sports fan.  When she is not cheering for OU, her entire family is rooting for the Cincinnati Reds.
